List out on a piece of paper or on a separate word document at least 4 ways teachers can assist students with disabilities in the transition process. Here is one area to get you started:
Conveying information about career and personal competencies to the rest of the IEP team
Identify post-school opportunities and services in which the student has access. Based on the student’s interests, where can they go to school (trade, community college, 4-yr college, etc.) or what work they can obtain and what agencies will support them in their process.
Monitoring progress on the transition plan
Conduct assessments across all life domains - this includes student’s strengths, needs, preferences, and interests. The results are used to help set appropriate goals.
